Personalized nurse coaching
and custom care kits

Nurse interventions for healthy behavior changes: Our nurse managers provide enrollees with personalized education and active coaching to make informed decisions about their health care. Enrollees receive one-on-one education, interactive monitoring, unlimited access to our 24/7 nurse-managed call center and assistance in setting personal health goals. They check in regularly with their nurses to receive guidance and assistance with their symptoms, medications, nutrition and other lifestyle issues.

Custom kits for healthy-self-management: Upon enrollment, each individual receives a self-care kit configured with items for his/her chronic condition. The kit contains educational materials, a pedometer, a walking tape, a welcome CD and other items designed to enhance knowledge and support the enrollee in meeting health goals.

Severity levels improve: Each enrollee is evaluated and placed into one of four severity levels for his/her chronic condition. The severity level defines which proprietary clinical pathway will be followed, based upon “best practice” protocols and National Institutes of Health guidelines. Through healthy self-management, severity levels decrease over time and complications are avoided.
