
Our program is uniquely constructed to improve health status and prevent costly complications. An essential ingredient is working with enrollees to set meaningful goals and then creating self-management paths for them to realize those goals.
Goal-setting and incentives: Enrollees set personal health goals with their nurses, ranging from weight loss and lower cholesterol to improved eating habits and daily exercise. When goals are achieved, enrollees receive gift certificates or other rewards to reinforce their positive lifestyle changes. As much as they appreciate the gifts, enrollees tell us that their renewed sense of independence and mastery of new behaviors are more important to them and their families.
