Measuring disease management health outcomes

Enrollees set personal health goals with their Focused Health nurse, ranging from specific test ranges (glucose meter, peak flow readings, etc.) to losing weight or quitting smoking.

Progress toward these goals is shared during frequent check-in phone calls and other contact with their nurses. In fact, the typical enrollee may have up to 40 customized “touch points” per year to help them stay on track with their care plan:

  • Up to 12 scheduled calls/year
  • Up to 10 “as needed” calls/year
  • Health newsletters, mailings, work-site educational sessions and other encounters

Through this highly personalized communication, significant health improvements have been tracked:

  • 21.6% reduction in average severity level for enrolled population after three program years
  • 37% reductions in HbA1c lab values for diabetic enrollees
  • 61% reduction of back/neck pain for enrollees who initially reported the high pain levels
