Focused Health Solutions is a national provider of customized health services for self-insured employers. During the late 1990s, the firm grew out of CM HealthCare Resources, a for-profit subsidiary of Children’s Memorial Medical Center which provided comprehensive home care services in the Midwest.

As interest in disease management emerged, Focused Health Solutions was formed to specifically address employers’ needs for innovative cost solutions. After a successful pilot program with ComEd (now Exelon) in 2000, Focused Health Solutions has broadened its customized services for chronic health conditions and contracts with self-insured employers throughout the U.S. Today the firm partners with 18 major employers, many of whom are the most admired companies in the world. Its success led to its acquisition by Linden LLC, a private equity firm specializing in the health care and life science sector.

As client partnerships are forged, we continue to clarify and deepen the business community’s understanding of active disease management and how it can be used as a proven cost solution in the workplace.
