A man’s diabetes was out of control and he was facing insulin shots until he joined the Focused Health Program. “Seven months ago my doctor was planning on placing me on insulin due to poor glycemic control. My numbers are so much better now. Every time I talk to my nurse I learn something new and it helps me with my sugar control.”
This enrollee met his personal health goals by losing weight; achieving blood glucose levels of 107-130 mg/dl and experiencing no new diabetes-related complications. He is happy with the support from his nurse and has not needed an ER, hospital or urgent care visit in the past twelve months.
This Spanish-speaking woman enrolled for her hypertension, but her lifestyle changes eventually affected her spouse, children and co-workers. Since enrolling, she has learned a lot from the educational materials and has changed her eating habits with a special focus on low-salt alternatives. Her family now spends more time together eating home-cooked meals that she boasts are healthier and better-tasting. She even shared her new approach with her in-laws when they visited from Mexico.
Her children support her “heart healthy” habits by walking with her to the park everyday. Both her blood pressure and cholesterol came down significantly and she has lost 25 lbs. She praises the Focused Health nurses for their help and their encouragement to use her blood pressure monitor and scale to track her progress.
One enrollee has been taking clonazepam for years to cope with panic attacks and his physician suggested that he switch to another medication. During a regular check-in call, his Focused Health nurse discovered that his daily caffeine intake was extremely high. He virtually eliminated caffeine from his diet, including his favorite soda pop, and found that his anxiety attacks stopped and he is sleeping better. After years of suffering from debilitating attacks, he has been panic-free for weeks.
Now he is working with his nurse to minimize headaches associated with the caffeine withdrawal, but he is confident that those will disappear over time. This enrollee is delighted that a major dietary change improved his behavioral condition and gave him a renewed sense of freedom in his life.
This enrollee came to the program reporting severe back and neck pain. His nurse manager taught him how to do stretching to avoid pain and encouraged him to use his pedometer. Now he stretches daily, especially prior to any yard work, household chores or golf, and reports being pain-free except for an occasional twinge.
An avid golfer, he recently played 18 holes for a fundraiser at his daughter’s school. “I felt absolutely no fatigue, no soreness and no pain. Last year at the same fundraiser I had to quit after 9 holes because I just could not get past the pain.” This year he is able to enjoy his favorite sport and credits the Focused Health Program for his well-being.
A man and his son both enrolled for asthma services through the Focused Health Program. His son improved dramatically through the one-on-one coaching provided by his nurse manager. According to his dad, “When we started with the program our nurse manager explained some key symptoms, things we never caught before. After a couple of months on the program it seems to have made a huge difference. It used to be that after a week of our son getting sick we would take him to the doctor and begin breathing treatments that would last three weeks. Now, after being in the program, we are better at watching for symptoms and reduced our son’s need for breathing treatments by 70-80%.”
And the man’s asthma is under better control as well: “I always thought I could tell what my own body was doing. But the indicators on the device notify me. I know something is coming up two weeks ahead of time and can react to it before it becomes a problem.”
